Crossroads Care Medway ~ Who Are We?

Crossroads Care Medway is a specialist voluntary organisation providing support to Carers in their own homes, giving Carers “time” to be themselves.

What Services do we provide?

Our service caters for those with critical and substantial care needs, including tailor-made care, where necessary, to meet the needs of each individual. This can include short term care following hospital discharge up to and including palliative end of life care.

Who can use our services?

Our service is available to those currently assessed by the Medway Social Care team and those who have been allocated financial funding by the Direct Payments team. There is also the opportunity to purchase our service on a private basis.

What Area Do We Cover

Crossroads Care Medway is an incorporated charity serving all of the Medway towns, including: Strood, Rochester, Chatham, Gillingham, Rainham and the more rural areas including the Hoo Peninsula.
